Background and Aim: Defense mechanisms facilitate emotional and interpersonal regulation and determine the socioemotional functionings of individuals. Suicidal ideation can be considered a means of escaping emotional dysregulation and interpersonal difficulties. This study aimed to determine the mediating role of emotional and interpersonal difficulties in the relationship between defense mechanisms and suicidal ideation in mothers of children with intellectual disabilities. Research Methods: The statistical population of this descriptive correlational study included all mothers of children with intellectual disabilities in Arak, Iran, in 2022. In total, 270 people were selected using the convenience sampling method. The data were collected using Defense Style Questionnaire-40, the Emotion Regulation Difficulty Scale, the Inventory of Interpersonal Problems-47, and the Suicide Cognitions Scale. Structural equation modeling was used to analyze the data. Results: The indirect path coefficients between mature defense mechanisms and suicidal ideation as well as immature defense mechanisms and suicidal ideation were significant. Discussions: Emotional and interpersonal difficulties play a mediating role in the relationship between defense mechanisms and suicidal ideation in mothers of children with intellectual disabilities.
